Final Forms Information for 2020-2021

Perry High School’s Athletic Department would like to advise parents/guardians of all 2020-2021 prospective athletes (Grades 7-12) of the following information:
1. FINAL FORMS REGISTRATION is now open for the 2020-2021 school year!  At your earliest convenience, please be sure to:
    a. Log in to your Final Forms account at perrymassillon-oh.finalforms.com
    b. SELECT ALL SPORTS that the student may be interested in participating for the 2020-2021 school year so that the additional athletic forms will appear.
        (This step is IMPERATIVE so that the coaches can identify and begin communicating with interested athletes.)
    c. Review and update the information currently appearing.
    d. Be sure to TYPE and SUBMIT your electronic signature where indicated on each form.
    e. Have your student log in to their Final Forms account, complete their portions, type and submit their electronic signature where indicated on each form.
2. PHYSICALS – Due to the CDC’s Covid-19 Limitations on Gatherings, Perry High School is unable to host its annual Physical Day.  If your student’s physical has expired, or is about to, we encourage you to make an appointment for your student to obtain a sports physical through their doctor’s office, Walk In Urgent Care, STATCARE, etc.  After completing Step 1, print out the very last form entitled OHSAA PreParticipationEvaluation Physical (for physicians). This step compiles all the answers you entered to the medical questions and prints the information onto a concise form that you will hand to the physician as well as a blank Physical Examination Form that the physician will complete, date, and sign.
 
3. Mail the completed original Physical Examination Form to the following address so that it can be entered into Final Forms and kept on file for auditing purposes:

Speak Life Virtual 5K Run/Walk

It’s another first! A “Speak Life Resiliency Virtual 5K Run/Walk”. We ask that you complete the 5K before July 4th. There is more information on the link provided. Proceeds to benefit the Coleman Professional Services (Stark County) & Perry High School and Edison Middle School SMART Labs. Those who register will get a race “tech” shirt and the option to purchase a participation medal. Click this link to register.

Career Wellness Center Closed

Although the Governor has declared private gyms/fitness centers and pools may open to the public on May 26. The Perry Wellness Center and Natatorium are owned and operated by the Perry Local Schools. As such, we must follow the directives established by the Governor and State/Local Health Departments regarding the opening/closing of schools.  Due to those directives, the Wellness Center and Natatorium will remain closed until further notice. 

Summer Computer Insurance Option

We have contacted the Worth Group and have arranged for optional insurance for the summer months (now through August 18th, 2020). If you previously purchased device insurance during the school year, your child’s device is already insured through August 18th, 2020. The rates are as follows:
5th–8th Grade Chromebooks for only $7.50.
9th–12th Grade Laptops for only $8.50.

Computer Drop Off Information

Computer drop off is available at the following dates and times at door 24 of Perry High School. When you arrive please stay in your car and call 330.477.8121 extension 4357 if no one is outside. We will ask you to put the device and charger in a plastic bag we provide.
Mon. May 18th 8 – 3.30 – Senior and exchanges only
Tues. May 19th 8- 3.30 – Senior and exchanges only
Wed. May 20th 12.30 – 8 – Senior only and last day for exchange
Thurs. May 21st 8- 3.30 – Senior, 8th, 4th, and 3rd grade
Fri. May 22nd 8- 3.30 – Senior, 8th, 4th, and 3rd grade

Tues. May 26th 8- 3.30 – 8th, 4th, and 3rd grade
Wed. May 27th 12.30 – 8 – 8th, 4th, and 3rd grade
Thurs. May 28th 8- 3.30 – 8th, 4th, and 3rd grade
Fri. May 29th 8- 3.30 – 8th, 4th, and 3rd grade

Plant a Garden

Start a garden this summer!! Free garden seeds will be handed out with Grab and Go lunches on Friday, May 8, 2020.

Band Fundraiser Pick up and Information

Dear Band Students and Parents: In February, many of you ordered Butterbraid products to support the Perry Band program.  With social distancing in place, it has been difficult for us to arrange product delivery.  On behalf of the Perry Bands, I apologize for this delay.  We now have a pick up date scheduled for May 15th, from 3PM-6PM near the PHS Wellness Center entrance.  If you ordered Butterbraid products, this will be the only opportunity to pick them up.  Unfortunately, we can’t store the items for you if you are unavailable on this date,  so please arrange for someone else to pick up your order if you are unavailable.  We have limited access to school buildings and this is the very best we can do.  We ask that you stay in your vehicle until one of our volunteers assists you.  Please avoid the main entrance to PHS (near the front steps) as we will be conducting our Uniform/Equipment return for HS Band members during this time. All orders can be picked up in the parking lot near the PHS Swimming Pool/Wellness Center Entrance.  Again, this will take place on May 15th, from 3PM-6PM.  Thank you for your continued support and I hope each one of you is doing well. – Ryan Smith
