Summer Physical Education forms are now available. Students may click on the link below to print out a form or high school students may pick up a form in the High School MAIN OFFICE.
Students are to take the completed and signed form (a parent/guardian signature is required) along with payment in the form of a check made out to ‘Perry Local Schools’ to Mrs. Fisher in the Main Office. Only completed forms with payment will be registered. Openings are on a first come basis.
May 6-8 will be an enrollment window for high school students only, after May 8th the enrollment is open to all students grades 8-11. The deadline for enrollment is May 22 at 2:00PM. NO LATE applications will be accepted.

Click on the link below to compute your final grade in any course. Remember that if you earn two F’s in the second semester you will fail the course for the year. Students failing a Math or English course MUST attend summer school to make up the course and credits. It is strongly suggested that you speak to your teacher if you are failing a course. The following courses/credits are required to graduate
PE- 1/2 credit (may be waived using the PE Waiver)
Health-1/2 credit
Math- 4 credits (Algebra I, Geometry, Algebra II, Applied Math or higher)
English- 4 credits (English 9,10,11,12)
Social Studies- 3 credits (US History, Government, one more credit of Social Studies)
Fine Art- 1 credit (Art, Speech, Choir, Band)
It is also recommended that students planning on attending college take 2 years of a World Language
Total credits for graduation – 21 credits
Students planning on taking a Stark State College dual credit course at Perry High School for the first time must complete the Stark State College online application . The student will create an account to request a PIN. Once the student has created an account, an online application must be completed. When completing the online application the student should choose ‘Fall 2015′ as the Admission Term and for ‘Application Type’ choose ‘Dual Enrollment High School St’. The student MAY NOT receive his/her Stark State ID number before March 13th.
All students must complete the Dual Enrollment Packet that is available in the Guidance Office. Students who just completed an online Stark State application (January 2015 on) will not have their Stark State ID number. Students may turn in the completed packet (include any test scores) to the Guidance Office without the Student ID # on or before March 13th.

Students who are taking or have taken a Stark State Dual Credit class at PHS do not need to reapply to Stark State while in high school. However, the student is required to write his/her Stark State College Student ID number on the Dual Enrollment Course Selection sheet and on the fall and spring enrollment forms. Failure to do so will result in the packet being returned to the student. The ID number would have been mailed to the student when he/she completed the application and can be found on the student’s Stark State College ID which was also mailed to the student upon enrollment. Last, the student ID can be found on the student’s My Stark State account accessed through the Stark State website. The number will start with the letter ‘S’.

Students who are taking French or Spanish Credit through KSU for the first time must complete the application, have taken the ACT or will be taking the ACT in April (March 13th is the regular deadline for ACT registration. go to www.actstudent.org) and take the Kent State Foreign Language Placement Test. The Foreign Language Placement Test will be administered at Perry High School on March 10th after school in the library. Students must sign up on the registration sheet found outside the Guidance Office.
If the student is not able to attend the March 10th testing date, he/she may call the Kent State Stark Campus testing Center to set up a testing time and date at the KSU Stark testing office.
In addition, the student must complete the Dual Enrollment packet found in the Guidance Office. All forms and packets for the Dual Credit Spanish and Dual Credit French courses ONLY, are due no later than May 1st. A written essay is no longer a part of the application.

The Compass test for Stark State Dual Enrollment classes at Perry High School has been rescheduled for March 10th. Current high school students must sign up on the Compass Registration Sheet outside the Guidance Office. High School students must report to the library by 2:15 and bring their social security number. If the Algebra Compass is being taken, students should bring their calculator and scratch paper.
High School students planning on taking Kent State University, Stark Campus French Dual Enrollment or Spanish Dual Enrollment at Perry High School for the first time are required to take the Foreign Language Placement Test. Current high school students must sign up on the Foreign Language Placement Test registration sheet outside the Guidance Office. The test will be offered after school in the library on March 10th. Testing will begin at 2:15. Students are to bring their social security number. Students are also required to complete the Kent State University on-line application. The application should be available on the KSU College Credit Plus website in early March.
Current Edison Intermediate School students will be taking the Compass on March 10th. Students are to register (call 330-477-3486 ext. 2801 and leave the student’s name) by March 9th at 2:00. Students are to bring their social security number and if the student is taking the Algebra portion of the test, he/she should bring a calculator. The doors will close and testing will begin at 3:30 on March 10th.
